or agency that's eligible to make purchases without paying GST, in order
to take your tax-exempt status, you'll need to provide us with the
following information by fax at the same time you place your order by

physical address.

This is to certify that the property and/or services ordered/purchased
hereby are being purchased by (Name of Provincial/Territorial-Government
Department or Institution) with Crown funds, and therefore are not
subject to Goods and Services Tax. (Signature of Authorized Official)
Mark your fax: "Attn: GST-Exempt."
